Dutch News · Athletic Physicals – 2020-21 Requirements

Due to the COVID-19 pandemic, the Michigan High School Athletic Association has implemented a one-year change to the athletic physical requirement for the 2020-21 school year.

If your child has a physical on file in the athletic office that is dated on or after April 15, 2019, a new physical is not required for the 2020-21 school year. This applies to any student who participated on an HPS-sponsored athletic team during the 2019-20 school year. In place of the physical, these students will need to complete and return the MHSAA Sports Health Questionnaire prior to participation in a practice/tryout. The form is available at the following link: MHSAA Health Questionnaire Please note that it is acceptable to complete, sign, and send this form electronically. If doing so, please email to dbecksvo@hollandpublicschools.org and include your child’s name in the title of the email.

Unfortunately, the Athletic Physical Night that is typically held in the spring was cancelled and will not be rescheduled.
